For Transitional Living SkillsTransitional Living Skills
Definition: Skills that adults use everyday to manage their lives. These equate to:
The Bridge Case Managers The Bridge Case Managers are dedicated mental health workers that work specifically with children/adolescents that are assigned to The Bridge. These Case Managers help families find, coordinate, and maintain services that are needed to help their child learn to successfully live independently. Growing Access Panel (GAP) GAP is comprised of caring community citizens who can assist your child in achieving the goal of independent living. GAP members insure that needed resources are not overlooked. Bridges Wrap Around Meetings These meetings include members of GAP. In addition, you will choose friends and family that you want included on this team. This team will identify strengths and needs that need to be addressed with your child. These identified strengths and needs will then become part of the treatment plan for your child. Bridges University The Bridge University meets once a week to teach your child Independent living skills. The Bridge University follows the Reinventing The Wheel Curriculum that was developed by SRS for just this purpose. One On One Support During one on one support, your child's The Bridge Case Manager and/or Attendant Care will reinforce lessons learned at The Bridge University. They will provide opportunities for your child to learn these skills with a hands on approach in the community. |
